In the event of a life threatening medical emergency, Cave Spring Church will attempt to contact the Parent/Guardian and the emergency contact provided during registration. If no contact is listed or reachable, appropriate medical personnel will be notified. 

Liability Clause for Awana Program at Cave Spring Baptist Church By registering my child for the Awana program at Cave Spring Baptist Church, I acknowledge and agree to the following: 1. Assumption of Risk: I understand that participation in the Awana program involves physical activities, games, and events that may carry inherent risks of injury or illness. I voluntarily assume all risks associated with my child's participation in these activities. 2. Release of Liability: In consideration of my child being permitted to participate in the Awana program, l, on behalf of myself, my child, and our heirs, release, waive, and discharge Cave Spring Baptist Church, its staff, volunteers, and affiliates from any and all liability, claims, demands, or causes of action arising from any injury, loss, or damage to my child or property, whether caused by negligence or otherwise, during participation in the Awana program. 3. Medical Authorization: In the event of an emergency, I authorize Cave Spring Baptist Church staff or volunteers to seek and obtain medical treatment for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred. 4. Acknowledgment of Rules and Conduct: I agree that my child will abide by the rules and guidelines of the Awana program as set forth by Cave Spring Baptist Church. I understand that the church reserves the right to dismiss any participant for behavior deemed unsafe or disruptive. By signing the registration form, I confirm that I have read, understood, and agree to this liability clause.
